*Physical Therapist Responsibilities:*
* Conduct thorough assessments of patients' physical abilities and needs.
* Develop personalized treatment plans based on individual patient goals.
* Implement therapeutic exercises and techniques to improve mobility and strength.
* Educate patients and their families on treatment plans and home exercise programs.
* Collaborate with case manager to ensure comprehensive patient care.
* Document patient progress and adjust treatment plans as necessary.
* Provide compassionate support to patients throughout their rehabilitation journey.
*Physical Therapist Requirements:*
* Valid California state license as a Physical Therapist.
* Strong knowledge of medical terminology and anatomy.
* Experience in patient care within clinical or home health settings preferred.
* Ability to develop effective treatment plans tailored to individual needs.
* Excellent communication skills for interacting with patients and families.
* Familiarity with acute care environments is a plus.
* Travel within a 30 mile radius may be required
