In this valuable role, the Clinical Supervisor is accountable for the supervision of PSW's and HSW's who are caring and making a difference for clients within the community. The Clinical UCP Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.

What you'll do:

  • Lead and support PSW's and HSW's to ensure practices comply with all legislated standards of practice and company internal protocols

  • Respond to and mentor staff, patients/families, and other team members with regular touchpoints about concerns from community

  • Provide ongoing leadership through regular huddles, face-to-face meetings and one-on-ones.

  • Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.

  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.

  • Engage patients in collaborative problem-solving by building trust, providing health education, and identifying their care needs, strengths, and goals

  • Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.

  • Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.

What you bring:

  • Current RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions

  • Minimum of 5 years' experience as a practicing Registered Nurse

  • Comprehensive patient health assessment skills are required

  • Possess an understanding of the principles of community nursing practice

  • Proven experience leading and managing a team

  • Knowledge of clinical documentation standards and practice guidelines is required

  • Excellent organizational, interpersonal, and teaching skills

  • Commitment to professional development in clinical practice that is relevant to home health care

  • Effective critical thinking and problem-solving skills

  • Must possess a valid driver's license and access to reliable transportation

  • Able to provide a current, clear Vulnerable Sector Screening (VSS)

  • C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset
