Nurse Practitioner

We are a rapidly expanding provider of innovative medical care for long-term and post-acute patients nationwide. We utilize advanced technology to identify at-risk patients and support timely, appropriate interventions.

Position:
Full-time Nurse Practitioner or Physician Assistant for on-site care in Skilled Nursing Facilities in Corsicana, Texas. No weekends, nights, call, or holidays required.

Responsibilities Include

• Conducting comprehensive health assessments and developing personalized care plans
• Managing chronic and acute conditions with timely interventions
• Ordering, interpreting tests, prescribing treatments within scope
• Providing resident and family education on health and prevention
• Collaborating with interdisciplinary teams and participating in quality improvement
• Maintaining accurate documentation and supporting organizational goals

Requirements

• Current Texas Nurse Practitioner license and board certification
• Geriatric care experience preferred; background in ICU, ER, Urgent Care, or Surgery advantageous
• Strong assessment, communication, and teamwork skills
• Proficiency with electronic medical records and healthcare technology
• Ability to travel occasionally within the region
