Job Summary
We are seeking a dynamic and compassionate Case Manager/Success Coach to empower clients and support their journey to self-sufficiency. In this role, you will serve as a dedicated guide, helping individuals navigate educational goals, develop personalized success plans, improve quality of life and connect with county-wide resources. Your energetic approach will foster a motivating environment where participants feel supported, inspired, and equipped to achieve their goals. Caseloads range varies depending on the nature of the service and acuity of assigned clients.
Duties
- Provide services in alignment with best practices, such as housing first, progressive engagement, whole family, and strengths-based approaches.
- Provide services in compliance with contractual requirements and established policies and procedures.
- Implement and adhere to workflows that promote high quality care and outcome-focused service delivery.
- Establish goal-based service plans, monitor progress through regular communication and engagement, and appropriately discharge according to program policies and procedures.
- Identify and link clients to resources and services, including for housing and tenancy, workforce development, education and vocational training, financial literacy, and youth focused supports.
- Coordinate direct financial support in the form of assistance and incentives.
- Provide advocacy, support, motivation and accountability for client goals.
- Maintain effective relationships with referral sources and service partners.
- Utilize designated tracking tools and database(s) to record client information, service transactions, and program expenditures.
- Advance the agency's organizational values: social justice focused, data driven, outspoken, collaborative, trauma-informed, investors.
- Perform other duties and manage other projects as assigned.
- Facilitate effective communication through public speaking and outreach efforts to promote available resources
- Collaborate with faculty, staff, and external partners to coordinate support services and recruitment initiatives
Qualifications
- At least 2-3 years of case management experience in an equivalent setting; Bachelors degree in a human services field (Health, Psychology, Sociology, Social Work etc) is desired but not required.
- Experience providing services in one or more content areas (housing stabilization, workforce development, youth/family supports).
- Experience working with individuals and families who face multiple barriers to achieving self- sufficiency and ensuring a trauma-informed approach to services.
- Experience working with government-funded programs or projects.
- Proficiency in Microsoft office-tools and the ability to learn and utilize one or more electronic platforms for service documentation and tracking.
*Pay will be comparable with your skills and experience. ($45k-$51.5k)
