Compensation:
- Salary: $30-$35/hr based on experience

Key Responsibilities:- Conduct patient assessments to determine their social service needs.
- Develop and implement personalized care plans in coordination with an interdisciplinary team.
- Provide counseling and emotional support to patients and their families.
- Advocate for patient needs within the facility and in the broader community.
- Maintain detailed and confidential patient records.
- Oversee discharge planning and coordinate with community resources to ensure continuity of care.
- Master's Degree in Social Work or a related field required
- Current LCSW certification in the state of SC
- Experience in long-term care health settings, particularly in social services.
- Strong understanding of case management and care coordination.
- Exceptional communication and interpersonal skills.
- Ability to work compassionately with a diverse patient population.
