Compensation:

• Salary: $30-$35/hr based on experience

• Conduct patient assessments to determine their social service needs.
• Develop and implement personalized care plans in coordination with an interdisciplinary team.
• Provide counseling and emotional support to patients and their families.
• Advocate for patient needs within the facility and in the broader community.
• Maintain detailed and confidential patient records.
• Oversee discharge planning and coordinate with community resources to ensure continuity of care.

• Master's Degree in Social Work or a related field required
• Current LCSW certification in the state of SC
• Experience in long-term care health settings, particularly in social services.
• Strong understanding of case management and care coordination.
• Exceptional communication and interpersonal skills.
• Ability to work compassionately with a diverse patient population.
