Job Title: Case Manager – ECM Program

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

Position Summary:
We are seeking a compassionate, organized, and motivated Case Manager to join our Enhanced Care Management (ECM) Program. This role focuses on supporting high-risk patients through care coordination, outreach, and ongoing follow-up to improve health outcomes and reduce barriers to care.

The ideal candidate is patient-centered, detail-oriented, and experienced in care coordination, social services, or managed care programs.

Key Responsibilities:

  • Conduct outbound and inbound patient outreach for ECM enrollment and follow-up
  • Assess patient needs and identify barriers to care (medical, behavioral, social)
  • Coordinate care with providers, clinics, and community resources
  • Assist patients with appointment scheduling, referrals, and follow-ups
  • Document all patient interactions accurately in the EMR and ECM tracking tools
  • Maintain assigned patient caseload and meet daily/weekly productivity goals
  • Collaborate with clinical teams, eligibility, referrals, and care coordinators
  • Support quality metrics, audits, and program compliance requirements
  • Maintain patient confidentiality and HIPAA compliance at all times

Qualifications:

  • Case management, care coordination, social work, or healthcare experience required
  • Experience with ECM, Medi-Cal, managed care, or population health programs preferred
  • Strong communication and interpersonal skills
  • Comfortable with high-volume outreach and documentation
  • EMR/EHR experience required
  • Bilingual (English/Spanish) strongly preferred
  • LVN, MA, or equivalent healthcare background a plus
