*About the Role:*
As a Physical Therapist, you’ll play a key role in helping our patients regain function and independence from the comfort of their own homes. You’ll be responsible for providing direct therapy, collaborating with physicians, educating patients and families, and leading the care team in delivering excellent outcomes.
*Your Responsibilities Will Include:*
* Conducting initial and ongoing comprehensive assessments, including OASIS
* Creating individualized treatment plans in collaboration with physicians
* Delivering hands-on therapy (e.g., exercise, massage, modalities) to support recovery and function
* Educating and supporting patients and their families in the home therapy program
* Supervising PTAs and paraprofessionals according to agency policy
* Collaborating with the care team and providing instruction to Home Health Aides as appropriate
* Participating in discharge planning and completing Physical Therapy Discharge Summaries
*What You’ll Need to Join the Team:*
* Florida Physical Therapy License
* Current BLS CPR certification
* At least one year of experience in a home health or community-based setting
* AHCA Level II Background Screening
* Experience with Wellsky (Kinnser) preferred

Pay: $65.00 - $110.00 per hour
