*Essential Functions:*
1 .Review pertinent evaluations and medical history. Review each student’s IEP prior to providing first treatment and preferably prior to the first day of school. Ensure that the physician’s prescription is current prior to the beginning of therapy. Periodically review the IEP to ensure that PT services are being implemented correctly
2.Assess informally and formally each child’s progress toward goal acquisition.
3.Document treatment including progress on daily session notes, quarterly progress notes, yearly evaluations and updating of IEPs. Write required reports documenting each child’s progress and current level of functioning, including strengths and needs. Submit treatment log notes according to VCLC timeline requirements.
4.Communicate and collaborate with families as needed to discuss the child’s progress and needs (including but not limited to beginning of year introduction, fall and spring parent meetings, ongoing updates, and support).
5.Communicate and consult with classroom team to provide regular updates on child’s progress in therapy and facilitate carry-over of physical therapy interventions and activities in the classroom.
6.Ensure all missed sessions are made up. SED requires 100% of sessions missed due to therapist absences be made up in a timely manner (30-day period). It is expected that each therapist will make every attempt to make up their missed sessions, however other staff may be required to assist. SED requires that EVERY attempt be made to make up sessions due to children’s legal absences.
7.Participate in the interdisciplinary team meetings, collaborating with the teacher, other therapists, and staff members to ensure that a comprehensive educational plan is in place to meet the child’s individual needs.
8.Perform such other tasks as may seem to be appropriate and requested by the CEO, CFO, Executive Program Director, Principal, Clinical Coordinator.
*Work Environment: *This job operates in a school environment. Activities usually take place in the classroom, in the gym, outside on the playground or in special activity rooms. This role routinely uses standard office equipment such as laptop computers, I-pads. printers/fax/scanner, telephone, and shredder.
*Physical Demands: *This role may be physically demanding. The employee is required to be non-sedentary. Must be able to stand; run, walk; sit; bend, climb stairs; balance; stoop, kneel, crouch; The employee must be able to lift an average pre-school age child (35 pounds) and school age child (50 pounds).
*Required Education and Experience: *
1. Bachelor of Science in Physical Therapy
2. Current NYSED registration as a Physical Therapist Preferred Education and
*Experience or Preferred Certifications:*
1. Doctor of Physical Therapy (graduate level first professional degree)
2. Experience and knowledge working with children with complex physical and communication needs.
*Additional Eligibility Qualifications:*
1. Must attend meetings including but not limited to opening day, staff conference days, staff meetings and Back to School Night.
2. Must maintain annual medical clearance and all mandated security clearances.
3. Must complete the required number of training hours as specified in the VCLC Staff Development Policies covering required OCFS categories.
4. Must complete mandated trainings as designated by VCLC administration, including but not limited to Discrimination & Harassment Training, Compliance Training, Safety Training, NCI Training, and any other Best Practice Trainings.
5. Must keep required Professional Certifications current.
6. Must be a mandated reporter
