Are you a skilled and compassionate *Physical Therapist* looking to make a real impact in patients' lives from the comfort of their homes? We are a growing *Home Health Agency* in *Lake Havasu City, Arizona*, providing integrated services including *Physical Therapy (PT), Occupational Therapy (OT), Home Health Aides (HHA),* and *Skilled Nursing (SN)*. We are currently seeking a dedicated *Home Health Physical Therapist* to join our team!
*Position: Home Health Physical Therapist (PT)*
*Location:* Lake Havasu City, AZ
*Status:* Full-Time / Part-Time / PRN options available
*Supervisory Role:* Supervision of a Physical Therapist Assistant (PTA) required
*What You'll Do:*
* Provide in-home physical therapy services to patients recovering from illness, injury, or surgery.
* Develop individualized care plans in coordination with our interdisciplinary team.
* Evaluate patient progress and adjust treatment plans as needed.
* Supervise and collaborate with a licensed Physical Therapist Assistant.
* Maintain timely and accurate documentation in compliance with agency and state guidelines.
*What We’re Looking For:*
* *Active Physical Therapy license in the state of Arizona (Required)*
* *Prior experience in Home Health setting (Preferred)*
* Strong communication and clinical skills
* Self-motivated, compassionate, and organized
* Willingness to travel within the Lake Havasu City area

Job Type: Full-time
Pay: $49.00 - $56.00 per hour
