QUALIFICATIONS :
Licensed or registered to practice as Occupational Therapist / Physical Therapist in the state of Virginia
Preferably at least two (2) year of experience as Home Health PT/OT
Valid Virginia Drivers License
Medicare Certified Home Health Care Agency, *HORIZONS HEALTHCARE SERVICES *, is looking for Home Health Physical Therapist /Occupational Therapist in Northern Virginia. We are serving the healthcare needs of our community by providing services needed in order to allow an individual to remain safely and comfortably in their home. The PT/OT under physician orders, is responsible for the treatment/management of clients needing PT services in the home setting. The therapist will provide in-home, one-on-one care to a predominately geriatric population. This home health position will give providers the opportunity to apply their therapy skills, while allowing for a flexible schedule. Responsibilities include proficiency in PT /OT management/treatments. The ability to work independently required. Excellent communication and interpersonal skills essential.
Job Description:
Records clinical/progress notes in the patient's clinical record weekly. Instructs the family in the client/patient's total speech therapy program and evaluates the home environment. Instructs other involved agency personnel in the care of client/patients receiving physical/occupational therapy. Plan for patient's discharge, prepares discharge instructions. Computer Skills for documentation and able to drive to Client’s home.
-Provide therapy services consistent with the patient’s Plan of Treatment.
-Observe patient during treatment, compile data and report patient’s progress.
-Act as a liaison and maintain contact with patients, family and Clinical Team to ensure effective care coordination.
-Provide hands-on care and education of the patient

\*Degree from an accredited Physical / Occupational Therapy program
\*Home Health experience a preferred
\*Current and unrestricted VA Physical Therapy / Occupational Therapy License
\*Good organizational and communication skills
