Provides primary care for a panel of patients using the medical home model of care, as part of an interdisciplinary team. Practices, promotes and implements current safety policies and procedures. Assesses safety needs of patients and recognizes opportunities to improve environmental and operational safety. Identifies quality improvement (QI) issues, participates in QI monitoring and evaluates clinical practice. Utilizes QI findings to improve quality of care. Provides leadership in adhering to and promoting practice based on current standards of clinical practice.
