Nurse Practitioner

Senior Community Care, a PACE (Program of All-Inclusive Care for the Elderly) provider, is seeking a Nurse Practitioner to deliver primary and preventive care to seniors in home, clinic, and community-based settings. You will conduct assessments, develop and manage care plans, prescribe medications, and collaborate closely with an interdisciplinary team of physicians, nurses, therapists, and social workers.
