*Responsibilities*
* Dispense prescription medications accurately while adhering to all legal and safety standards, including HIPAA regulations.
* Provide expert medication counseling to patients, explaining proper usage, potential side effects, and answering questions to promote understanding and adherence.
* Conduct comprehensive patient assessments to determine appropriate medication therapy, including evaluating medical histories and current health conditions.
* Manage pharmacy inventory efficiently by ordering supplies, monitoring stock levels, and ensuring proper storage of medications, including compounding medications when necessary.
* Utilize pharmacy software and electronic health record (EHR) systems to document patient interactions, medication records, and inventory management activities precisely.
* Administer immunizations and perform IV infusions following aseptic techniques while maintaining compliance with safety protocols.
* Collaborate with healthcare providers on medical management plans for diverse patient populations, including pediatrics and those requiring critical care or pain management.
*Skills*
* Extensive knowledge of LTC (Long-Term Care) pharmacy operations and hospital-based pharmacy practices.
* Proven experience in critical care settings with a strong understanding of physiology, anatomy, and medical terminology.
* Demonstrated proficiency in pharmacy software systems and EMR/EHR platforms for accurate documentation and communication.
* Skilled in compounding medications using aseptic techniques and managing IV insertions for infusion therapies.
* Strong patient service skills with the ability to perform thorough patient assessments and medication administration procedures.
* Experience in inventory management, medication safety protocols, and hospital pharmacy workflows.
* Certification in immunizations and familiarity with HIPAA compliance standards; previous pharmacy residency or clinical pharmacy experience is highly desirable.
