Social Worker

Sanford Health is seeking a compassionate Social Worker to join our Healthcare & Medical Services team. In this role, you will assess patient psychosocial needs, coordinate care plans, provide counseling, and connect patients and families with community resources. You will collaborate closely with physicians, nurses, and interdisciplinary teams to support safe discharges and continuity of care across clinics, hospitals, and senior living facilities.
