Inpatient Case Manager RN

LifeBridge Health seeks an Inpatient Case Manager RN to coordinate comprehensive care for hospitalized patients across the continuum. In this role, you’ll complete clinical and psychosocial assessments, develop and manage evidence-based care and discharge plans, and collaborate with physicians, nurses, and allied health teams to ensure safe, timely transitions. You will conduct utilization review, engage patients and families in education, connect them with community resources, and support quality and process improvement.
