Physical Therapist

Saint Mary of Nazareth Hospital, a leading community healthcare provider in Chicago, IL, is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based, patient-centered physical therapy to a diverse adult and geriatric population across inpatient and outpatient settings. You will be responsible for performing comprehensive evaluations, developing individualized treatment plans, and delivering therapeutic interventions aimed at restoring function, improving mobility, reducing pain, and promoting optimal physical health. Working collaboratively with physicians, nurses, case managers, and other allied health professionals, you will contribute to interdisciplinary care planning, participate in discharge planning, and ensure smooth transitions of care. Daily responsibilities include conducting objective assessments, documenting findings and progress in the electronic medical record, educating patients and families on home exercise programs and injury prevention, and adhering to all regulatory and hospital quality and safety standards. You will help mentor students or new staff as needed and engage in continuous professional development through in-services, trainings, and participation in quality improvement initiatives.
