Speech Language Pathologist, Home Care, Bronx

VNS Health, a leading provider of home and community-based healthcare services in New York City, is seeking a compassionate and skilled Speech Language Pathologist to join our Home Care team serving patients in the Bronx. In this role, you will travel to patients’ homes and community settings to assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders across diverse adult and geriatric populations. You will perform comprehensive evaluations, develop individualized, evidence-based treatment plans, and deliver goal-directed therapy that supports safety, communication, and quality of life in the home environment. Working closely with an interdisciplinary team—including nurses, rehabilitation therapists, physicians, social workers, and caregivers—you will provide education and training to families on communication strategies, home exercise programs, and safe swallowing techniques. You will document care in a timely and accurate manner using electronic medical record systems, participate in case conferences, coordinate care transitions, and advocate for appropriate services and equipment. This position offers a dynamic and autonomous workday in the Bronx community, with a caseload that reflects a wide range of medical and neurological conditions, such as stroke, dementia, Parkinson’s disease, traumatic brain injury, and complex medical comorbidities.
