Working closely with physicians, nurses, case managers, and other rehabilitation professionals, you will participate in interdisciplinary care planning, provide clear clinical documentation in the electronic medical record, and offer ongoing patient and family education on exercises, precautions, and functional strategies. You will be responsible for conducting gait and balance training, therapeutic exercise, manual therapy, and functional mobility training, as well as recommending and training in the use of assistive devices and durable medical equipment. You will monitor patient progress, adjust treatment plans as needed, and contribute to safe discharge planning and continuity of care.
As part of a mission-driven community hospital, you will be expected to demonstrate strong clinical judgment, cultural sensitivity, and excellent communication skills while managing a caseload efficiently in a fast-paced environment. You will uphold hospital policies, quality and safety standards, and regulatory requirements, including accurate, timely documentation and participation in performance improvement initiatives. You will also have opportunities for professional growth through in-service education, mentorship, and continuing education supported by the hospital.
This full-time position typically involves a hybrid work environment focused primarily on field/on-site clinical care within the hospital and affiliated outpatient settings, with some potential for remote documentation or telehealth follow-ups as appropriate. Flexibility to work some weekend or holiday rotations may be required to support patient needs.
