Patient Care Assistant - Day Or Night Shift Available

Schedule: 12-hour shifts | Days, Nights, or Weekends

Situated on a peaceful, wooded 26-acre campus, Nexus Neurorecovery Center Conroe offers the RISE Program, a comprehensive neurological rehabilitation program for adults recovering from severe traumatic brain injury (TBI), stroke, spinal cord injury, and other neurological diagnoses.

This campus is also home to the Life Care Program, which provides long-term residential care for individuals who have sustained serious, life-altering injuries and require ongoing clinical and behavioral support.

The Patient Care Assistant (PCA) provides hands-on care, supervision, and rehabilitation support to adult patients recovering from neurological injuries or living with long-term care needs. Working under the supervision of licensed nurses and rehabilitation professionals, the PCA helps patients with daily routines, therapeutic activities, and behavioral support in a compassionate, structured environment designed to promote recovery and independence.

• Provide direct care and assistance with Activities of Daily Living (ADLs) such as bathing, dressing, feeding, mobility, transfers, and grooming.
• Observe, monitor, and accurately document patient behavior, participation, and vital signs.
• Support therapeutic, rehabilitative, and recreational programming to encourage patient engagement and progress.
• Assist in implementing individualized care and behavioral support plans under clinical supervision.
• Maintain a safe, organized, and therapeutic environment, ensuring adherence to patient care protocols and safety procedures.
• Respond to crises or behavioral escalations using approved de-escalation and intervention techniques to maintain safety.
• Collaborate with nurses, therapists, case managers, and rehabilitation specialists to ensure coordinated and holistic care.

• De-escalation Training required within 30 days of hire
• CPR / BLS Certification required
