Physical Therapist

Altru Rehabilitation Hospital, part of Altru Health System, is seeking a compassionate and patient-centered Physical Therapist to join our Healthcare & Medical Services team in our integrated rehabilitation environment. In this role, you will assess, plan, and deliver evidence-based physical therapy interventions to a diverse caseload of patients with orthopedic, neurologic, post-surgical, and medically complex conditions across the continuum of care. You will complete comprehensive evaluations, develop individualized treatment plans, set measurable functional goals in collaboration with patients, families, and the interdisciplinary team, and provide skilled interventions focused on restoring mobility, improving strength and endurance, reducing pain, and maximizing safety and independence in daily activities. You will regularly reassess patient progress, adjust plans of care, and clearly document all findings, treatments, and patient education using our electronic medical record system, adhering to regulatory and professional standards. As part of Altru’s collaborative, community-focused culture, you will participate actively in interdisciplinary care conferences, coordinate closely with physicians, nurses, occupational and speech therapists, case managers, and other team members, and contribute to safe and timely discharge planning and transition of care. You will educate patients and caregivers on home exercise programs, safe mobility and fall prevention strategies, use of assistive devices, and lifestyle adaptations that support long-term health and wellness. The role also includes mentoring and precepting students as appropriate, engaging in ongoing professional development and evidence-based practice initiatives, and supporting quality improvement projects that enhance patient outcomes and the patient experience.
