Altru Rehabilitation Hospital, part of Altru Health System, is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services department. In this patient-centered role, you will evaluate, plan, and provide evidence-based physical therapy interventions to individuals across the lifespan who are recovering from illness, surgery, or injury, or who are managing chronic conditions. You will complete comprehensive musculoskeletal and neuromotor assessments, develop individualized treatment plans, and deliver therapeutic exercises, manual therapy, gait and balance training, and functional mobility interventions designed to restore function, improve quality of life, and promote independence. Working collaboratively within Altru Health System’s integrated care model, you will partner daily with physicians, nurses, occupational and speech therapists, case managers, and other members of the interdisciplinary team to coordinate care and ensure safe, timely progression toward patient goals. You will educate patients and families on home exercise programs, precautions, safe mobility, and strategies for long-term self-management, tailoring communication to each patient’s abilities, learning style, and cultural background. Documentation of evaluations, daily notes, progress reports, and discharge summaries will be completed accurately and promptly in the electronic health record, in accordance with regulatory, payer, and Altru Health System standards. This role offers the opportunity to work with a diverse caseload that may include orthopedics, neurology, post-surgical rehabilitation, cardiopulmonary conditions, and general medical deconditioning within an inpatient rehabilitation hospital environment. You will participate in care conferences, contribute to discharge planning, and make recommendations for adaptive equipment, assistive devices, and community resources that support safe transitions of care. You will also play an active role in falls prevention, mobility initiatives, and quality improvement efforts aimed at enhancing patient outcomes and the overall rehabilitation experience.
