Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a licensed Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based, patient-centered rehabilitative care to individuals recovering from illness, injury, or surgery in an acute inpatient rehabilitation setting. You will evaluate patients, develop individualized plans of care, and deliver therapeutic interventions aimed at maximizing mobility, function, independence, and quality of life.
Working in a dynamic, interdisciplinary environment, you will collaborate closely with physicians, nurses, occupational and speech therapists, case managers, and other team members to coordinate comprehensive treatment plans and ensure smooth transitions of care. Responsibilities include performing detailed physical and functional assessments, setting measurable clinical goals, providing hands-on therapy, educating patients and families on home exercise programs and safety, and documenting all care accurately and timely in the EMR in accordance with regulatory and hospital standards.
You will be expected to integrate the latest clinical research and best practices into your treatments, contribute actively to team conferences and care planning, and participate in performance improvement initiatives that support superior patient outcomes.
