Working closely with an interdisciplinary team of physicians, nurses, occupational therapists, speech therapists, case managers, and other rehabilitation professionals, you will collaborate to establish functional goals, monitor progress, and adjust care plans based on clinical outcomes and patient response. Daily responsibilities include conducting therapeutic exercise, gait and balance training, neuromuscular re-education, functional mobility retraining, transfer training, patient and family education, and discharge planning with appropriate home exercise programs and durable medical equipment recommendations.
You will document all evaluations, daily treatment notes, progress reports, and discharge summaries accurately and in a timely manner using our electronic medical record system, ensuring compliance with hospital, state, and federal regulatory and billing requirements. As part of our culture of integrity and clinical excellence, you will integrate current research and best practices into your care, participate in quality improvement initiatives, and contribute to the advancement of rehabilitation services within the hospital.
