MDS Coordinator (RN)

The Emerald Peek Rehabilitation and Nursing Center is seeking an experienced and detail-oriented MDS Coordinator (RN) to join our leadership team. This role is critical to ensuring accurate resident assessments, regulatory compliance, and optimal reimbursement while supporting exceptional resident care.

Position Summary

The MDS Coordinator is responsible for the timely and accurate completion of MDS assessments in accordance with federal and state regulations. This role works closely with the interdisciplinary team to ensure care plans reflect resident needs and support quality outcomes.

MDS Coordinator (RN) Key Responsibilities
  • Complete, review, and submit MDS assessments accurately and timely in compliance with CMS and state regulations

  • Coordinate the interdisciplinary care plan process

  • Ensure documentation supports MDS coding and reimbursement

  • Monitor Medicare, Medicaid, and insurance case-mix accuracy

  • Participate in audits and surveys; address deficiencies related to MDS

  • Educate and collaborate with nursing and clinical staff on documentation requirements

  • Track assessment schedules and ensure compliance with PPS and OBRA guidelines

MDS Coordinator (RN) Qualifications
  • Active RN license in good standing (NY)

  • Prior MDS Coordinator experience in a skilled nursing facility (required)

  • Strong knowledge of RAI process, MDS 3.0, and CMS regulations

  • Experience with PDPM preferred

  • Excellent organizational, communication, and analytical skills

  • Ability to work collaboratively with interdisciplinary teams

Salary: Up to $130,000 a year
