MDS Coordinator RN

Medina Center for Rehab & Nursing is Now hiring an RN MDS Coordinator!

We are currently seeking an experienced and detail-oriented RN MDS Coordinator to join our team. If you are passionate about quality care and regulatory compliance, we would love to meet you!

• RN license in good standing (RN preferred).
• Experience with MDS 3.0, care planning, and RAI processes in a long-term care setting.
• Knowledge of Medicare/Medicaid regulations and reimbursement guidelines.
• Excellent organization, communication, and computer skills.
• RAC-CT certification preferred (or willingness to obtain upon hire).

• Complete and coordinate accurate and timely MDS 3.0 assessments in compliance with federal and state regulations.
• Develop and update individualized care plans in collaboration with the interdisciplinary team.
• Ensure appropriate documentation supports coding and reimbursement accuracy.
• Submit MDS assessments and track CMS submissions to ensure error-free processing.
• Participate in care planning meetings, QA audits, and survey readiness.
• Educate and support clinical staff in understanding MDS and RAI processes.
