POSITION SUMMARY:
The Clinical Field Supervisor RN will collaborate with the Clinical Director and Clinical Care Manager to oversee and coordinate skilled nursing care for patients in need of home care services. This position ensures that care is provided in accordance with physician orders, relevant laws and regulations, and organizational policies.
RESPONSIBILITIES:
Clinical Care:
- Provides direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments. Maintains and updates schedules of all disciplines in a patient's home.
- Consistently assesses and evaluates the patient's status, goals, and revises the plan of care as necessary. Communicates adjustments to plan of care to all team members in the appropriate time frame to support consistent delivery of care. Complete authorization modifications in a timely manner, where needed.
- Implement preventative, therapeutic, and rehabilitative procedures and maintain communication regarding patient care with the physician, supervisory personnel, and other professional staff involved in patient care.
- Monitors and manages patient medications; maintains an accurate medication profile; coordinates prescription refills with physicians and pharmacies and picks up medication refills as needed.
- Educate patients and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician. Recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
- Supervises Home Health Aide, LPN, and RN to evaluate performance and quality of care.
- Accepts responsibility for assignments as qualified, ensuring appropriate training and demonstrated competency.
- Use supplies and equipment effectively and efficiently. Maintains equipment per agency policy.
- Manage an assigned patient case load including tracking of patient status when transferred, following up with other healthcare facilities providing care to patients, resuming care of patients when appropriate
- Adheres to HIPAA laws and maintains patient confidentiality.
- Execute other additional duties as assigned by the Clinical Director /Clinical management, such as performing field supervision of care team members as required by agency policies.
- Completes and submits accurate, timely clinical notes including all OASIS documents, visit authorization requests, physician orders, field supervision reports, quality assurance visit reports and all other clinical documentation in accordance with Innovive company policies.
Communication and Meetings:
- Maintains consistent communication with physicians and other agencies to ensure continuity of care and implementation of a comprehensive care plan. Collaborates as part of the interdisciplinary care team.
- Participates in scheduled clinical case management meetings, as well as vendor and referral meetings as required. Document meeting outcomes in the patient's clinical record.
- Coordinates with pharmacies, insurance companies and other service providers as needed to facilitate timely patient care.
Supervisory partnering:
- Conduct field supervision of nursing visits in a clinical management capacity while collaborating with the Clinical Director and Clinical Manager to assess the effectiveness of patient care, identify performance concerns and trends.
- Conduct quality assurance visits to ensure that care provided is reasonable, necessary and in accordance with patient specific plan of care
- Perform case load reviews with field nurses to ensure documentation supports the need for services / quick notes describe communication to care team and provider routinely and to report abnormal findings.
- Maintain and update documentation trackers for field supervision, quality assurance and case load reviews.
- Ensures the clinical team is adhering to all regulations and policies while conducting patient care in the home.
- Ensures communication and collaboration with other members of the multidisciplinary team (HHA, Rehab services).

REQUIRED QUALIFICATIONS:
- Graduate of an accredited school of professional nursing
- Current RN license through the State Board of Nursing and meets one of the following criteria:
- RN with a Bachelor's degree in nursing and one year of related working experience
- RN with a diploma or Associate's degree with two years related work experience
- Successfully passed National Council Licensure Examination (NCLEX)
- Minimum of 1 year of clinical home health experience
- Comply with accepted professional standards and principles
- Valid driver liceense with an automobile that is in good working order, or other means of reliable transportation
- Good physical stamina, must provide pre-employment physician's health clearance including a negative TB test and/or CXR and other tests as required by the organization's policy
- Possesses and maintains current CPR certification
- Positive references from nursing school, previous (or current) employers and/or professional peers
PREFERRED EDUCATION AND EXPERIENCE:
- Bachelors in Nursing (BSN) from an accredited institution
- Continuing Education Requirements:
- Agency personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, agency personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All agency personnel must attend mandatory education programs.
PHYSICAL DEMANDS AND WORK ENVIRONMENT:
- Frequently required to stand.

- Continually required to use hands and fingers.
- Frequently required to climb, balance, bend, stoop, kneel or crawl.
- Occasionally required to lift/push light weights (less than 25 pounds).
- Occasionally required to lift/push light weights (greater than 25 pounds).
- Moving, lifting, or transferring of patients may be required on occasion.
- Frequent exposure to bloodborne and airborne pathogens or infectious materials.
