Substance Use Disorder (SUD) Coordinator
Job Summary/Overview:
To maintain the coordinated delivery of substance abuse prevention, promotion and education services to the patients we serve, community groups, schools and businesses; and to do other work as required.
Responsibilities: Include but are not limited to:
* Evaluate needs and design and implement substance abuse prevention forums and programs. Schedule SUD/MAT service appointments.
* Perform appointment reminder calls and perform follow up calls for missed appointments.
* Complete client paperwork including intakes and screenings. Maintain client level reporting system and perform data entry, as needed.
* Complete required external and internal program reports within timely manner.
* Maintain appropriate documentation in patient's
