Description

Essential Duties and Responsibilities

In this role you will be:

  • Assist patients/residents with mobility, including walking, repositioning, transfers, and use of mobility aids (wheelchairs, walkers, canes, gait belts, lifts).
  • Support safe transfers between bed, chair, wheelchair, toilet, and other surfaces using proper body mechanics and approved techniques.
  • Monitor and report changes in mobility, strength, balance, or pain to nursing staff.
  • Encourage patient independence and participation in mobility activities as appropriate.
  • Follow individualized care plans and mobility protocols established by nursing or therapy staff.
  • Ensure mobility equipment is clean, properly maintained, and used correctly.
  • Maintain a safe environment to reduce fall risks.
  • Provide respectful, compassionate care while preserving patient dignity and privacy.
  • Accurately document mobility assistance and observations per facility policy.
  • Collaborate with nurses, physical therapists, and other healthcare team members.

Qualifications

Required:

  • Education: High school graduate or equivalent
  • Experience:
    • Experience assisting patients with mobility and transfers preferred.
    • Knowledge of safe patient handling techniques and fall prevention.
    • Ability to follow care plans and safety guidelines
  • Licensure/Certification:
    • Two years of Nursing Assistant experience preferred or CNA certification (required).
    • BLS certification required or obtained within 1 month of hire

Shifts: Monday - Friday 9am - 5:30pm
