In this role, you will conduct comprehensive speech, language, cognitive-communication, voice, and swallowing evaluations, develop individualized treatment plans, and deliver evidence-based interventions that promote safety, independence, and functional outcomes. You will collaborate closely with nurses, physical and occupational therapists, physicians, patients, and family caregivers to ensure coordinated, patient-centered care, while educating patients and caregivers on therapy techniques, compensatory strategies, and home exercise programs.
You will be responsible for managing a home-health caseload, traveling to patient residences, assessing home environments, and adapting therapy to real-life activities and patient goals. You will document assessments, treatment sessions, progress notes, and care plans accurately and timely within the electronic medical record, complying with all regulatory, payer, and CenterWell documentation standards. Ongoing reassessment, outcome tracking, and communication with the broader care team will be central to your contribution to patient safety and quality outcomes.
