Home Health Speech Language Pathologist

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to provide compassionate, in-home evaluation and treatment for adult and geriatric patients. In this role, you will assess speech, language, cognitive, and swallowing disorders, create individualized care plans, and deliver evidence-based interventions that support safety, autonomy, and quality of life. You will collaborate closely with an interdisciplinary team, maintain accurate clinical documentation, educate patients and caregivers, and contribute to continuous quality improvement in a culture grounded in integrity and excellence.
