Physical Therapist (New Grad Mentor Program)

FOX Rehabilitation is seeking a dedicated Physical Therapist to join our New Grad Mentor Program within our clinician-owned and operated private practice specializing in geriatric rehabilitation. In this role, you will deliver evidence-based, patient-centered physical therapy services to older adults in their homes and community settings, focusing on improving safety, mobility, function, and overall quality of life. As part of our structured new graduate mentorship, you will receive one-on-one clinical guidance, regular case reviews, and access to ongoing education designed to support your transition from student to confident, independent clinician. You will be responsible for performing comprehensive evaluations, developing and implementing individualized plans of care, providing therapeutic exercises and manual therapy interventions, and delivering gait, balance, and functional mobility training tailored to the specific needs and goals of each client. You will document all care using an electronic medical record system, apply current evidence-based practices and outcome measures, and collaborate closely with physicians, caregivers, and the broader healthcare team to ensure coordinated, high-quality service.
