Occupational Therapist

CenterWell Home Health seeks an Occupational Therapist to deliver high-quality, in-home therapeutic services that enhance patient autonomy and quality of life. In this role, you will complete comprehensive assessments, develop and implement individualized treatment plans, and provide evidence-based interventions to promote safety, independence, and functional outcomes. You will collaborate closely with an interdisciplinary team, educate patients and caregivers, document care accurately, and contribute to quality and safety initiatives in a culture grounded in compassion, integrity, and excellence.
