Sevita is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services team, providing person-centered care in home and community-based settings. As a Physical Therapist with Sevita, you will work with individuals of all ages and abilities, including those with complex medical, neurological, and rehabilitative needs, helping them maximize independence, mobility, and quality of life. In this role, you will conduct thorough evaluations, develop individualized treatment plans, and deliver evidence-based interventions tailored to each person’s goals, strengths, and preferences. You will provide therapeutic exercises, manual therapy, gait and balance training, neuromuscular re-education, and functional mobility training in real-life environments such as the home and community. You will also recommend and train individuals and caregivers in the use of assistive devices and adaptive equipment, ensuring safety, comfort, and long-term success. Collaboration is central to how we work at Sevita. You will be an integral member of an interdisciplinary care team, partnering with nurses, physicians, occupational and speech therapists, behavioral health professionals, and direct support staff to coordinate holistic care. You will educate families and caregivers, provide clear communication regarding progress and goals, and advocate for services and supports that enable each person to live life to the fullest.
