Nurse Practitioner or Physician Assistant

Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant to perform comprehensive in-home health assessments, chronic disease management, and preventive care for diverse members. You will collect detailed health histories, conduct physical exams, review medications, and document findings in our EMR to support quality and risk adjustment programs.
