*What You’ll Do*
* Evaluate patients in the home setting and develop individualized treatment plans
* Provide skilled physical therapy interventions to improve mobility, strength, balance,
safety, and function
* Educate patients, families, and caregivers on exercises, mobility strategies, and fall
prevention
* Monitor progress toward goals and modify treatment plans as needed
* Collaborate with physicians, nurses, therapists, and office staff to coordinate care
* Complete accurate and timely documentation
* Support patients in achieving the highest possible level of independence at home

Qualifications
* Active Utah Physical Therapist license
* Valid driver’s license
* Reliable transportation
* Ability to travel throughout assigned territory
* Strong clinical, communication, and documentation skills
Preferred
* Home health experience
* Residence in Phoenix or the East Valley, including Chandler, Gilbert, Mesa, or nearby communities
