RN Case Manager

Crossbridge Hospice seeks an RN Case Manager to provide compassionate, patient-centered end-of-life care in home and community settings. In this role, you’ll manage a caseload of hospice patients, complete thorough assessments, and develop individualized plans of care focused on comfort, dignity, and quality of life. You will coordinate with our interdisciplinary team—physicians, social workers, chaplains, and aides—to ensure seamless clinical, emotional, and spiritual support.
