Conviva Senior Primary Care is seeking an Associate Medical Director to help lead our senior-focused primary care practices within the Healthcare & Medical Services department. Conviva Care Centers specialize in primary care for seniors, delivering comprehensive, coordinated health services designed around the unique needs of older adults across the southeastern United States. This role is ideal for a clinically grounded physician leader who is passionate about quality, patient-centered care, and sustainable healthcare delivery. As an Associate Medical Director, you will provide clinical leadership and oversight to support our physicians, advanced practitioners, and care teams in delivering high-quality, evidence-based care to a predominantly Medicare and senior population. Working closely with site Medical Directors and operational leadership, you will help shape and refine Conviva’s care model, aligning daily practice with organizational goals around exceptional patient experience, improved outcomes, and responsible use of healthcare resources. You will participate in the development and implementation of clinical protocols, pathways, and standards of care tailored to geriatric primary care, with a strong focus on chronic disease management, preventive care, and functional preservation for older adults. Using data-driven insights, you will monitor key performance indicators such as quality metrics, hospitalizations, readmissions, and patient satisfaction, and you will help design and lead quality improvement and population health initiatives that translate into better outcomes and more coordinated care. In this role, you will collaborate with physicians, nurses, social workers, care coordinators, and practice operations teams to optimize clinic workflows, strengthen interdisciplinary collaboration, and ensure that care remains compassionate, timely, and accessible. You will provide mentorship and coaching to clinicians, support their professional development, and foster a culture of teamwork, accountability, and continuous learning. Limited direct patient care is expected, allowing you to maintain clinical skills and model best practices while dedicating substantial time to leadership responsibilities. The Associate Medical Director will also help ensure compliance with regulatory and payer requirements, particularly those relevant to Medicare and value-based care programs. You will contribute to the design and execution of initiatives that support population health management, risk stratification, and proactive outreach for high-risk seniors. Additionally, you will partner with Conviva leadership to align clinical strategies with broader organizational priorities, reinforcing our commitment to patient-centered, community-oriented, and outcome-driven care.
