LPN FT Day

Good Samaritan is seeking a compassionate LPN FT Day to join our Healthcare & Medical Services team, providing holistic, patient-centered care to underserved communities. As an essential member of our interdisciplinary care team, you will deliver direct nursing care during daytime hours, including conducting and documenting patient assessments, obtaining and recording vital signs, administering medications and treatments per provider orders and organizational protocols, and assisting with minor procedures and point-of-care testing. You will support providers in managing acute and chronic conditions, reinforce care plans, provide patient and family education on disease management, medications, and wellness, and promote preventive care consistent with our mission of improving community health outcomes. In this full-time role, you will coordinate patient flow for daily clinics, prepare exam rooms and equipment, maintain infection control and safety standards, and ensure accurate and timely documentation in the electronic health record. You will collaborate closely with physicians, advanced practice providers, RNs, medical assistants, social workers, and behavioral health professionals to address the physical, emotional, and social needs of patients, many of whom may face barriers to accessing care. You will participate in care huddles, contribute to quality improvement initiatives, and support population health efforts such as immunization campaigns, chronic disease registries, and follow-up outreach. This position requires strong clinical skills, attention to detail, reliable follow-through, and the ability to work effectively with diverse populations with compassion and cultural humility. The ideal candidate demonstrates integrity, clear communication, and a commitment to trauma-informed, strengths-based care. You will use your practical nursing expertise to triage phone calls and walk-in concerns within your scope, escalate issues appropriately, and respond to urgent patient needs in a calm and professional manner.
