Social Worker, Home Health-PRN

CenterWell Home Health is seeking a compassionate Social Worker, Home Health – PRN to provide high-quality, patient-centered care in the comfort of our patients’ homes. In this role, you will collaborate with an interdisciplinary team of nurses, therapists, and physicians to support patients and their families in managing medical, emotional, and social needs related to illness, recovery, and long-term health conditions. You will conduct psychosocial assessments, help identify barriers to care, and develop person-centered care plans that promote patient autonomy, safety, and an improved quality of life. Working closely with patients and caregivers, you will provide counseling, crisis intervention, and education about community resources, financial assistance programs, and support services that can help patients remain safely at home. You will coordinate referrals to community agencies, hospice or palliative care when appropriate, and assist with advance care planning and end-of-life discussions in a respectful and culturally sensitive manner. As a key member of the home health team, you will document timely and accurate visit notes, maintain regulatory and ethical standards, and participate in case conferences to review patient progress and adjust plans of care. This PRN position offers a flexible schedule with visits based on patient needs and staffing, making it ideal for an experienced home health or medical social worker seeking autonomy and variety in daily work.
