In this role, you will help patients stay in their homes safely by improving strength, coordination, and the ability to perform daily tasks. You will evaluate each patient’s environment, recommend equipment, and design treatment plans that focus on real-life function instead of just clinical exercises.
*Occupational Therapist | OTR/L Responsibilities*
* Complete in-home occupational therapy evaluations and treatments.
* Assess activities of daily living, mobility, and home safety.
* Develop individualized care plans based on patient goals.
* Provide hands-on therapy to improve independence and function.
* Recommend adaptive equipment and environmental modifications.
* Communicate with nurses, therapists, and providers to coordinate care.
* Document visits accurately in EMR.
*Occupational Therapist | OTR/L Qualifications*
* Active Minnesota Occupational Therapist license.
* Valid driver’s license and dependable vehicle.
* Comfortable working independently in the field.
* EMR experience preferred.
* Strong communication and patient education skills.
