Sapphire Rehabilitation & Care Center is seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have a strong commitment to improving the quality of life for patients across various settings, including clinics, skilled nursing facilities, and acute care environments. This role involves working with diverse populations, including geriatrics and pediatrics, to develop individualized treatment plans that promote independence and enhance daily living skills.
*Responsibilities*
* Conduct comprehensive evaluations of patients' physical, emotional, and social needs to determine appropriate treatment plans.
* Implement therapeutic interventions aimed at improving patients' functional abilities and overall well-being.
* Collaborate with interdisciplinary teams to ensure cohesive care and effective discharge planning.
* Provide education and support to patients and their families regarding treatment goals and home exercise programs.
* Document patient progress accurately and maintain compliance with all regulatory standards.
* Participate in case management activities to coordinate care across various settings, including inpatient and outpatient services.
* Stay current with best practices in occupational health and therapy techniques through ongoing professional development.
*Experience*
* A degree in Occupational Therapy from an accredited program is required.
* Valid state licensure or eligibility for licensure as an Occupational Therapist.
* Experience in geriatrics, pediatrics, or acute care settings is preferred but not mandatory.
* Familiarity with case management processes and discharge planning is advantageous.
* Strong interpersonal skills with a focus on patient-centered care.
* Ability to work collaboratively within a multidisciplinary team environment while maintaining a high level of professionalism.

Job Type: Full-time
Pay: $42.51 - $51.19 per hour
