In this role, you will perform comprehensive evaluations, develop and implement personalized plans of care, and deliver hands-on interventions and therapeutic exercise programs. You will utilize best-practice treatment approaches, including manual therapy, therapeutic exercise, neuromuscular re-education, balance and gait training, and patient education. You will document all care accurately and efficiently in our electronic medical record system, track patient progress using objective outcome measures, and adjust treatment plans based on clinical findings and patient goals.
You will collaborate closely with a dynamic, interdisciplinary team of clinicians and support staff who share a commitment to excellence, innovation, and patient-centered care. You will communicate regularly with referring providers and other healthcare professionals to coordinate care and ensure optimal outcomes. As part of BreakThrough Physical Therapy’s culture of continuous improvement, you will stay current with emerging research and contribute to a learning environment that encourages clinical growth and innovation.
This position offers the opportunity to build strong therapeutic relationships with a diverse patient population, including post-surgical, orthopedic, sports-related, and chronic pain cases. You will educate patients and their families on home exercise programs, injury prevention, lifestyle modifications, and self-management strategies that support long-term success. You will manage your caseload effectively to maintain high-quality care while meeting productivity and documentation expectations.
