We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will provide high-quality care to patients in the outpatient environment. This role involves developing individualized treatment plans, implementing therapeutic interventions, and collaborating with healthcare professionals to ensure optimal patient outcomes.
*Responsibilities*
* Conduct comprehensive assessments of patients' physical abilities and limitations.
* Develop and implement personalized care plans that address specific patient needs.
* Utilize various therapeutic techniques, including trigger point therapy and orthopedic rehabilitation.
* Monitor patient progress and adjust treatment plans as necessary.
* Provide education to patients and their families regarding treatment options and home care strategies.
* Collaborate with interdisciplinary teams to coordinate discharge planning and ongoing care.
* Maintain accurate documentation of patient evaluations, progress notes, and care plans in accordance with medical terminology standards.
*Requirements*
* Master (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program.
* Current state licensure or eligibility for licensure as a Physical Therapist.
* Proficiency in medical terminology and the ability to develop effective care plans.
* Strong communication skills for interacting with patients, families, and healthcare teams.
* Knowledge of orthopedic conditions and rehabilitation techniques.
* Ability to work collaboratively within a team-oriented environment.
