We’re looking for a compassionate and dedicated  Medical Director to lead our  Home-Based Primary Care (HBPC)team, bringing exceptional healthcare directly to Veterans in the comfort of their own homes.

As our HBPC Medical Director, you’ll guide a team of skilled professionals—nurses, social workers, therapists, and support staff—united by one purpose: helping Veterans live with dignity, independence, and the highest possible quality of life. This is more than medicine; it’s about forming relationships, fostering trust, and making a difference where it matters most.

What You’ll Do:

  • Provide clinical leadership and direction for an interprofessional team.

  • Deliver comprehensive, in-home primary care to Veterans with complex health needs.

  • Collaborate across disciplines to create care plans that prioritize respect and compassion.

  • Lead initiatives that strengthen our connection to the community we serve.

Who You Are:

  • A licensed physician (M.D. or D.O.) with board certification in Family Medicine or Internal Medicine.

  • Experienced in geriatric, palliative, or home-based care (preferred but not required).

  • A leader who values teamwork, empathy, and service.
