LPN (Licensed Practical Nurse) Day / Night

The Center at CenterPlace is Greeleys premier short-term rehabilitation facility, designed to help patients recover after surgery, illness, or hospitalization. Our center features 54 private suites, state-of-the-art rehabilitation equipment, and care delivered in a comfortable, hotel-like environment.
We are looking for *team-oriented Licensed Practical Nurses (LPNs)* who take pride in patient care, communicate well with staff and families, and are focused on helping patients recover and move forward. We are looking for LPNs to work day & night shift currently.
What Youll Do
As an LPN in our center, you will play a key role in the day-to-day care of our patients. You will work closely with nurses, CNAs, physicians, and therapy teams to ensure patients receive safe, effective care during their recovery.
Your responsibilities will include:
* Administering medications and treatments according to physician orders
* Accurately documenting medications, treatments, and patient status
* Monitoring patients for changes in condition and responding appropriately
* Communicating changes in condition, lab results, or concerns to the clinical team according to facility policy
* Educating patients and families about medications, treatments, and care plans when appropriate
* Ensuring each patients care plan is followed and implemented
* Supervising and coordinating the work of Certified Nursing Assistants (CNAs)
* Supporting a safe, organized, and patient-focused care environment
What Were Looking For
* Current *Licensed Practical Nurse (LPN) license* in the state of Colorado
* Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required
* Strong communication and teamwork skills
* Attention to detail and commitment to patient safety
* A patient-focused mindset and strong work ethic
