Avera Health is seeking a compassionate and patient-centered Occupational Therapist to join our Healthcare & Medical Services team within our regional health system. Based within our collaborative, multidisciplinary environment, you will provide evidence-based occupational therapy services to patients across the continuum of care, helping them achieve greater independence, safety, and quality of life.
In this role, you will evaluate patients with a wide range of physical, cognitive, and psychosocial conditions, develop individualized treatment plans, and deliver therapeutic interventions aimed at improving functional performance in daily activities, work, and leisure. Working closely with physicians, nurses, physical therapists, speech therapists, social workers, and other members of the care team, you will ensure coordinated, patient-focused care consistent with Avera Health’s values of compassion, hospitality, and stewardship.
Key responsibilities include conducting comprehensive assessments; setting measurable, functional goals in collaboration with patients and families; implementing therapeutic activities and interventions; and providing patient and caregiver education on adaptive techniques, energy conservation, safety, and use of assistive devices. You will fabricate or recommend splints and adaptive equipment as appropriate, collaborate on discharge plans, and provide recommendations for home or community modifications that support long-term independence.
The Occupational Therapist will document all assessments, treatment sessions, progress notes, and care plans accurately and timely in the electronic medical record, adhering to professional, regulatory, and payer standards. You will participate in team conferences, case discussions, and quality improvement initiatives, contributing clinical expertise to optimize patient outcomes and support best-practice protocols.

This is a full-time position typically based on-site within an Avera Health facility, with potential for some hybrid services depending on departmental needs and patient populations.
