Occupational Therapist

Avera Health is seeking a compassionate and patient-centered Occupational Therapist to join our Healthcare & Medical Services team within our regional health system serving communities across the Upper Midwest. In this role, you will provide evidence-based occupational therapy services to patients of varying ages and diagnoses, helping them regain function, promote independence, and improve their quality of life. Working collaboratively within an interdisciplinary team of physicians, nurses, rehabilitation professionals, and other care providers, you will perform comprehensive evaluations, develop individualized treatment plans, and deliver therapeutic interventions aligned with each patient’s goals and abilities. As an Occupational Therapist with Avera Health, you will assess functional abilities in areas such as activities of daily living, motor skills, cognition, vision and perception, and psychosocial functioning. You will select and implement appropriate therapeutic exercises, modalities, and adaptive techniques, including the use of assistive devices, splinting, and environmental modifications. You will educate patients and families on home programs, safety strategies, and adaptive strategies to support a safe transition across care settings, whether in acute care, inpatient rehabilitation, outpatient clinics, or other care environments within our health system. You will be responsible for maintaining accurate and timely documentation in the electronic medical record, tracking progress toward measurable goals, and communicating effectively with the care team to coordinate holistic treatment plans. You will contribute to discharge planning, provide recommendations for equipment and community resources, and support continuity of care.
