Oak Street Health is seeking a dedicated Physician to join our Healthcare & Medical Services team, delivering comprehensive primary care to adults on Medicare in underserved communities. As a Physician with Oak Street Health, you will provide high-quality, patient-centered care rooted in prevention, chronic disease management, and strong patient relationships. You will manage a panel of adult and senior patients, conducting thorough evaluations, developing individualized care plans, and coordinating with an integrated care team to improve health outcomes and reduce avoidable hospitalizations. In this role, you will perform routine and acute care visits, conduct annual wellness exams, manage multiple chronic conditions, and ensure evidence-based, guideline-driven treatment. You will collaborate closely with nurses, medical assistants, care managers, social workers, and other clinicians in a team-based model that emphasizes communication, continuity, and accessibility for patients. You will use electronic health records to document encounters, track quality metrics, and follow up proactively on diagnostic results, referrals, and care gaps. You will contribute to a culture that is patient-centered, inclusive, and collaborative, actively participating in case conferences, quality improvement initiatives, and population health efforts. You will engage patients and their families in shared decision-making, provide clear education about diagnoses and treatment options, and help address social determinants of health by connecting patients with community resources. The position offers consistent clinic hours with limited inpatient responsibilities, allowing you to focus primarily on outpatient primary care.
