Physical Therapy Assistant

Avera Health, a regional health system based in Sioux Falls, SD, is seeking a compassionate and skilled Physical Therapy Assistant to join our Healthcare & Medical Services team. In this role, you will work under the direction and supervision of licensed physical therapists to provide high‑quality, patient‑centered rehabilitative care across a variety of settings, helping individuals regain mobility, improve function, and enhance their overall quality of life. As a Physical Therapy Assistant at Avera Health, you will implement individualized treatment plans, guide patients through therapeutic exercises and functional activities, and use approved modalities to support recovery from injury, surgery, or illness. You will carefully observe and document patient responses, progress, and tolerance to interventions, communicating changes to the supervising physical therapist and participating in adjustments to the plan of care. You will ensure accurate and timely documentation in the electronic medical record in accordance with regulatory, departmental, and professional standards. You will collaborate closely with physical therapists, nurses, physicians, and other members of the interdisciplinary care team to coordinate therapy services, support safe patient mobility, and promote continuity of care. With a strong focus on education, you will instruct patients and their families or caregivers in home exercise programs, safe body mechanics, use of assistive devices, and strategies that support independence and prevent further injury. You will help maintain a safe, clean, and well‑organized treatment environment and ensure that equipment is used and stored properly.
